Affirmation Counselling holds information about you when you contact us or if we are contacted on your behalf. We aim to make contact with you within 48 hours of contact If we receive no further contact your information will be kept for 28 days and then deleted or destroyed.
Information will be collected during an Initial Assessment. This information may include your name, date of birth, address, email address, telephone number(s), gender, brief reasons you are coming to us, GP details, employment status, ethnicity, age, sexuality, disability and religion.
Any personal and private information will be held separately from any subsequent notes made of counselling sessions by your counsellor. All counselling notes and paperwork are safely and securely stored. We will keep your data for 3 years after you have completed your work with us or as agreed when you contract at the Initial Assessment.
As per the contract signed before commencing counselling, data may be shared if the counsellor has any safeguarding concerns. Safeguarding concerns can include the client, vulnerable adults and children. This will be explained and contracted with you at the Initial Assessment.
